BMJ Case Reports Published online: [please include Day Month Year] doi:10.1136/ bcr-2012-008086 DESCRIPTION The case of a patient with primary hyperparathyroidism with classic and severe skeletal involvement has been previously reported. Briefly, it was the case of a 63-year-old woman who presented with multiple repeated fractures, the latest of which were of both femoral shafts. Primary hyperparathyroidism was diagnosed based on elevated calcium with unsuppressed parathyroid hormone (PTH) levels. The excess PTH secretion was localised to a right inferior parathyroid adenoma. She eventually underwent parathyroidectomy with documented decline in calcium and PTH levels. A preoperative skeletal survey radiograph was performed on November 2008, 3 months before parathyroidectomy. Among the findings was a lytic lesion of the fourth metacarpal of the right hand, compatible with osteitis fibrosa cystica. Repeat x-ray performed on April 2012 or 38 months after parathyroidectomy showed complete remineralisation of the previously noted lytic lesion. The area previously occupied by the osteitis fibrosa cystica now appears hyperdense indicative of bone formation (figure 1). Her fractures have also healed and there has been no recurrence of any fracture since the surgery. She has also been ambulatory since then. Areas of the bone affected by osteitis fibrosa cystica are known to resolve, with radiographs showing hyperdensities within 3 months of successful parathyroidectomy. These radiographical changes are also accompanied by histological changes in bone, showing an osteoblastic or healing response.

Unusual presentation of primary hyperparathyroidism: report of three cases

BACKGROUND Primary hyperparathyroidism is an endocrinopathic condition characterized by hypersecretion of parathyroid hormone. Excess parathyroid hormone results in an altered state of osseous metabolism involving bone resorption and tissue change known as osteitis fibrosa cystica, which is the end stage of primary hyperparathyroidism. Osteitis fibrosa cystica is associated with the development...

Scintigraphic evaluation of multifocal osteolytic lesions in a patient with primary hyperparathyroidism: A case report

Osteitis fibrosa cystica is the classic patognomonic form of skeletal disease in hyperparathyroidism that characterizes with decreased cortical bone thickness compared to increased cancellous bone. We present a case of 52-year old female patient with osteolysis of the left calf on radiographic images. The bone scan detected multiple focal pathological accumulations in the skull, left tibia, bot...

A Reference Finding Rarely Seen in Primary Hyperparathyroidism: Brown Tumor

Primary hyperparathyroidism is an endocrinopathy which is characterized with the hypersecretion of parathormone. During the progress of the disease bone loss takes place due to resorption on the subperiosteal and endosteal surfaces. Brown tumor is a localized form of osteitis fibrosa cystica, being part of the hyperparathyroid bone disease. It is rarely the first symptom of hyperparathyroidism....

Tetany despite normocalcaemia and normomagnesaemia following parathyroidectomy.

Two patients with long-standing hyperparathyroidism due to a parathyroid adenoma are described. Both had severe osteitis fibrosa cystica and a proximal myopathy. Both were treated with alfacalcidol (2 micrograms/day) prior to and following parathyroidectomy, and infused with calcium gluconate following the operation. Plasma total and ionized calcium and magnesium concentrations were maintained ...

Severe Hyperparathyroidism with Secondary Osteitis Fibrosa Cystica and Brown Tumors Mimicking Bone Metastasis

Introduction: The classical presentation of primary hyperparathyroidism, osteitis fibrosa cystica, has become very rare nowadays. This rarity makes it sometimes misdiagnosed leading to disastrous outcomes. Methods: We present a case of an undiagnosed primary hyperparathyroidism with severe osteitis fibrosa cystica and brown tumors that was first misdiagnosed as having bone metastasis. Conclusio...
